What Is Polyvagal-Informed Care?

If you work in social work, nursing, counseling or another healthcare field, you've likely encountered patients or clients whose emotional responses seem disproportionate to the situation at hand. Understanding why that happens, and how to respond in a way that promotes healing rather than escalating distress, is at the heart of polyvagal-informed care.
The Science Behind It: Polyvagal Theory
Polyvagal theory was developed by neuroscientist Stephen Porges, PhD, and introduced in 1994. It is a scientific framework focused on how the autonomic nervous system continuously organizes the body's physiological state in response to environmental demands. Rather than simply describing stress responses, polyvagal theory explains how autonomic regulation shapes the body's ongoing capacity to maintain stability, support health and adapt to changing conditions.
At its core, the theory proposes three primary states the nervous system cycles through based on perceived safety:
- Ventral vagal state (safety): The nervous system supports calm, social engagement and connection.
- Sympathetic state (danger): The body mobilizes for fight or flight.
- Dorsal vagal state (life threat): The body shuts down or immobilizes as a survival strategy.
These states shift dynamically and often outside of conscious awareness, influencing how people feel, think and relate to others.
What This Means for Trauma
Polyvagal theory has significant implications for understanding trauma and PTSD. According to the Institute for Functional Medicine, PTSD symptoms may be understood as the product of a reconditioned autonomic nervous system that developed during extreme or repeated exposure to threat. Patients with PTSD often experience abnormal fluctuations in autonomic states, cycling between fight-or-flight behaviors, immobilization and dissociation without an intervening calm state.
Research has also found that patients with PTSD tend to show decreased heart rate variability, which reflects an autonomic state oriented toward mobilization and survival rather than rest and connection. Polyvagal theory provides a neurophysiological framework for understanding these patterns, shifting how clinicians view trauma responses from character flaws or overreactions to measurable physiological adaptations.
How Polyvagal Theory Informs Clinical Practice
Polyvagal-informed therapy uses the principles of polyvagal theory to guide clinical interventions that help clients move out of chronic stress states and into greater capacity for social engagement, rest and healing. Key techniques include:
- Co-regulation: The therapist or care provider uses their own calm, regulated presence to help the client shift from a state of sympathetic arousal or dorsal vagal shutdown toward the ventral vagal state of safety and connection. This is one of the foundational principles of polyvagal-informed care.
- Mindfulness and grounding: Clients learn to stay present and recognize their own physiological cues, such as shallow breathing or muscle tension, before those cues escalate into full distress responses.
- Polyvagal mapping: Clients are helped to understand their own autonomic patterns and identify strategies to return to a regulated state.
- Somatic and expressive approaches: Art therapy, music therapy and somatic experiencing provide additional pathways for clients to access and process emotions at a body level.
According to the IFM, contemplative practices like mindfulness, yoga, tai chi and controlled breathing may also activate the vagus nerve and support recovery from trauma by increasing what researchers call respiratory sinus arrhythmia (RSA), a physiological marker linked to the ventral vagal complex.
Why This Matters Beyond the Therapy Room
Polyvagal-informed care isn't limited to mental health therapy. Its principles are relevant across healthcare and social services settings. For nurses, it informs how to approach patients in distress. For social workers, it explains why a client may appear shut down or explosive in ways that feel disconnected from the immediate situation. For anyone working with people who have experienced trauma, understanding the nervous system's role in behavior creates space for more compassionate and effective care.
The vagus nerve, as the central component of the parasympathetic nervous system, helps regulate mood, immune response, digestion and heart rate. When vagal tone is weak due to illness or trauma, clients may chronically cycle between anxiety and shutdown. Polyvagal-informed care offers tools to help strengthen that regulatory capacity over time.
